and username associated with that social networking service, as well as any information
you make public using that social networking service or that the social networking service
allows us to access.

• Device Information: When you visit the Site or use our Services, we collect Personal

Cookie ID, unique identifiers (e.g. MAC address and UUID) and other information which

Please note that we process any health information only under the instructions of the health
care provider which is providing you with health care services. We do not exercise our discretion
